What is an anal fissure?

An anal fissure is a small cut or tear in the thin skin lining the anus. In Marathi it is often called फिशर or परिकर्तिका. It usually happens after passing a hard or large stool. Sometimes it follows loose motions. It is common in young adults, and women can get it after childbirth.

Common signs

• Sharp, cutting pain while passing stool
• Burning or throbbing pain that lasts minutes to hours afterwards
• A little bright red blood on the stool or tissue
• Fear of going to the toilet because of the pain
• Sometimes a small skin tag near the tear

Acute and chronic fissure

An acute fissure is new, usually less than six weeks old. It often heals on its own with the right care.

A chronic fissure has lasted longer than about six weeks, or keeps coming back. The muscle around the anus (the internal sphincter) goes into spasm. This tight muscle reduces blood flow to the tear, so it does not heal. The pain makes people hold back stool, which makes stool harder. It becomes a cycle.

First treatment: simple care and medicines

Most patients start with simple steps:

• Eat more fibre: bhakri, vegetables, fruit, dal
• Drink enough water through the day
• Take a stool softener if the doctor advises
• Sit in a tub of warm water (sitz bath) for 10 to 15 minutes, two or three times a day
• Use the ointment prescribed by the surgeon, which helps relax the muscle and ease pain

When is laser treatment advised?

If a chronic fissure does not heal after proper medical treatment, the surgeon may advise a procedure. One option is a laser sphincterotomy. The laser is used to release a small part of the tight internal muscle. This reduces spasm, improves blood flow and allows the tear to heal. A skin tag, if present, may be removed at the same time.

For many patients, this brings relief from pain within days, with minimal bleeding. Most return to their routine quickly. As with any procedure, there are some risks, and results vary. Your surgeon will explain these clearly.
