What is a perianal abscess?
A perianal abscess is a pus-filled boil next to the anus. In Marathi it can be described as गुदद्वाराजवळील गळू. It usually begins when a small gland inside the anal canal gets blocked and infected. The infection spreads into the soft tissue around it and forms a painful swelling.
Common signs
- Constant, throbbing pain near the anus that gets worse day by day
- A hot, red, tender swelling
- Pain while sitting, walking or passing stool
- Fever, chills or feeling unwell
- Pus discharge if it bursts
Some abscesses are deep and may not show much swelling from outside. They can still cause severe pain and fever.
Why is this urgent?
An abscess is a closed pocket of pus. Antibiotics alone usually cannot clear it. If it is left, the infection can spread, cause high fever, and in rare cases become serious. People with diabetes need extra care, because infection can spread faster. That is why a perianal abscess usually needs drainage soon.

How is it treated?
The surgeon examines the area and decides if drainage is needed. Drainage is done under anaesthesia. A small opening is made so the pus can come out. This usually gives quick relief from pain. The cavity is cleaned and dressed. You may be given antibiotics and pain medicines.
After drainage
- Take sitz baths (sit in warm water) as advised
- Get dressings changed on the dates given
- Eat soft, high-fibre food and drink enough water
- Keep your blood sugar controlled if you have diabetes
The link between abscess and fistula
In some patients, after an abscess heals, a small tunnel stays between the inside of the anus and the skin. This is called a fistula (भगंदर). Signs include a small opening that keeps leaking pus or fluid. This is not a failure of the drainage. It is how the problem sometimes develops. That is why follow-up visits after an abscess are important. If a fistula forms, the surgeon will discuss treatment, which may include laser fistula closure.
